With the rise in inhalation allergies over the last few decades, dysfunctions of nasal respiration have in crea singly become the object of scien tific studies. It was primarily Messerklinger and Albegger [1] who suggested and carried out such studies for hyperactive rhinopa\u0002thies. In spite of the suggestions made by Wolf [2], a medication therapy has yet to be developed, leaving symptomatic treatment for reduction of the nasal turbinates as the most helpful alternative.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"> Introduction: The greatest percentage of en largement of the turbinates by far, and therefore obstruction of nasal respiration, is caused by the turbinate body. For that rea - son, an effective surgical procedure should concentrate on this area. It should not just aim at eliminating the hyperplasia, but it should also seek to prevent future swelling.<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"476\" height=\"147\" src=\"https:\/\/getremed.gr\/wp-content\/uploads\/2022\/02\/bipolar-lavida.jpg\" alt=\"\" class=\"wp-image-5974\" srcset=\"https:\/\/getremed.gr\/wp-content\/uploads\/2022\/02\/bipolar-lavida.jpg 476w, https:\/\/getremed.gr\/wp-content\/uploads\/2022\/02\/bipolar-lavida-300x93.jpg 300w\" sizes=\"auto, (max-width:767px) 476px, 476px\" \/><\/figure><\/div>\n\n\n\n<p class=\"wp-block-paragraph\"> Materials and methods: Swelling of the turbinates resulting from other diseases or brought on by medication should be ruled out and treated at the cause. In particular, sinusitis or allergic rhinopathy should be considered. Such symptoms also accom\u0002pany metabolic diseases (diabetes) or hor\u0002monal disorders (hypothyroidism). Swel - ling of the turbinates also occurs as a side\u0002effect of medication, especially with contra\u0002ceptives (estrogen level), anti-hypertensi\u0002ves and anti-epileptics. One should also  inquire as to the possible misuse of tume\u0002scent nasal drops. The extraordinary ten\u0002dency for frequent recurrence is the grea\u0002test nuisance with any treatment of the nasal turbinates. In the end attempts to prevent re currence lead to increasingly radical operations \u2013 long-lasting suc cess  is often not forthcoming. The bipolar Radio fre quen cy Volume Re duc tion (RaVoR\u2122) of the turbinates is based on other con - siderations:<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"217\" height=\"122\" src=\"https:\/\/getremed.gr\/wp-content\/uploads\/2022\/02\/insulation-1.jpg\" alt=\"\" class=\"wp-image-5978\"\/><\/figure><\/div>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"217\" height=\"122\" src=\"https:\/\/getremed.gr\/wp-content\/uploads\/2022\/02\/insulation2.jpg\" alt=\"\" class=\"wp-image-5980\"\/><\/figure><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">1.A high recurrence rate is inevitable with any procedure. For this reason, treat\u0002ment should pose as little stress as pos\u0002sible; it should be easy to do and able to be repeated.  <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">2.Tissue is heated and destroyed only bet\u0002ween and around the two electrodes.  <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">3.The procedure can be carried out under local anesthesia.  <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">4.The treatment can be performed as an out patient procedure during office hours.  <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">5.The procedure takes only a few minutes.  <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">6.There is no down-time and no restriction in the patients\u2019 quality of life <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patient preparation: A surface anesthe\u0002tic is applied (e.g. a cotton strip soaked in  lidocaine 4% or a spray), optionally mixed with 50% vasoconstrictor such as xylome\u0002tazoline or adrenaline. The patient may leave the treatment room for approx. 10 minutes, but should remain under supervi\u0002sion. The decongested turbinates allow an evaluation of the anatomy of the nasal pas\u0002sage, in particular of the bony part. It is often possible to recognize the places where an enlargement of the turbinates obstructs respiration. These areas should receive spe\u0002cial attention during treatment.  Local anesthetic is injected into each tur\u0002bi na te, 2-3 ml on each side, followed by a  3-5 minutes break to allow depots of anes - thetic to disperse. In the meantime, i.e. be - fore use, the protective insulation of the \u201cBinner\u201d radiofrequency probe has to be in - spected before use to make sure that the mu cosa remains intact (see Figure 2). Be - gin ning at the head of the turbinate and repeating the procedure in the middle part and, if indicated in the posterior part (me\u0002dial surface), the probe is inserted com\u0002pletely with its thin protective insulation  parallel to the bone (see Figure 3).  Following a correct placement of the probe, radiofrequency ener gy is applied. During treatment visible changes of the mucosa are not likely. How ever, the application should be stopped if a slight white discoloring of the mucosa is ob served. To treat pronoun\u0002ced areas of hyperplasia subsequent punc\u0002tures may be required. There should be only very little bleeding. If desired, to mana\u0002ge hemorrhage, the tips of the probe may be applied on the surface (not inserted) and the mucosa coagulated.  <\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"222\" height=\"159\" src=\"https:\/\/getremed.gr\/wp-content\/uploads\/2022\/02\/monitor-prasino.jpg\" alt=\"\" class=\"wp-image-5982\"\/><\/figure><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Post-operative care: Specific treatment is not required. Experience has shown that tamponades are not necessary. Pain killers (e.g. paracetamole or diclofenac) may be given and nasal saline douches (three times a day) recommended. Patients should ab - stain from smoking, heavy physical activity and excessive nose blowing. In the days fol\u0002lowing surgery nose breathing may tem\u0002porarily be obstructed due to post-operative swelling. If required, a spray reducing nose swelling (e.g. xyloetazoline) may be pres\u0002cribed. Healing is completed after 3 to 6 weeks. The procedure may then be repeated on any remaining hyperplasia.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"> References: <em>1.
